Initial Ketamine Evaluation: $575 self-pay (50% deposit required). Blue Cross Blue Shield covers evaluation; copay/deductible/co-insurance apply. In-office and Zoom ketamine sessions: $650/session self-pay; $350/session BCBS (covers 1h only). Integration therapy: $250/30-minutes or $350/45-minutes self-pay. BCBS covers integration therapy. Financing up to 12 months available through CareCredit. HSA/FSA accepted.
