With insurance: $300 cash upfront + copay/per infusion for mood disorders, $550 cash upfront + copay/per infusion for chronic pain. IM Ketamine without insurance: $500/per infusion for mood disorders. With insurance: $150 cash upfront + copay/per infusion. Psych clearance $250, Medical clearance $150, Post IV follow-up $150. Medication management initial evaluation $250, follow-ups $150.
